Official form
The form approved by the canton, mandatory to notify a rent increase — and, in several cantons, the initial rent. An increase announced without it, or received too late, is void.
The form must reach the tenant at least ten days before the start of the notice period preceding the intended term date, state its reasons, and name the rate and index the new rent rests on. The date that counts is receipt, not the postmark.
